游舒涵副教授實驗室(The S.H.Y. Lab) 主要研究主軸包括腫瘤免疫微環境(tumor immune microenvironment, TIME)與個人化醫療(personalized medicine)。本實驗室運用高通量空間轉錄體技術,結合 AI 輔助多重免疫組織化學分析(AI-guided multiplex immunohistochemistry),解析腫瘤微環境中的空間組成與細胞互動特徵。透過此平台,可重建腫瘤微環境中具功能意義的免疫微區(immuno-clustering),並進一步分析這些區域內不同細胞族群之間的空間鄰近性與交互作用,進行高解析度的空間生物學研究。

腫瘤免疫微環境由腫瘤細胞、腫瘤浸潤性免疫細胞、癌症相關纖維母細胞、腫瘤血管系統及細胞外基質等多種成分共同構成。這些細胞與非細胞成分可協同調控腫瘤免疫逃脫、促進腫瘤生長與轉移,並影響病人對治療的反應。因此,針對病人腫瘤組織及其周邊組織中的免疫細胞組成與空間分布進行系統性分析,將有助於建立標準化的腫瘤免疫微環境分析平台,並進一步應用於精準醫療與個人化治療策略的發展。

游博士實驗室所建立的腫瘤微環境研究平台,可應用於犬貓腫瘤及相關疾病治療機轉之探討,也可整合於實驗室長期關注的口腔鱗狀細胞癌(oral squamous cell carcinoma, OSCC)研究。藉由分析 OSCC 病人腫瘤微環境的空間組成與免疫細胞互動模式,本實驗室致力於評估其作為免疫治療療效預測指標的潛力。相關研究成果將有助於臨床醫師依據不同病人的腫瘤免疫微環境特徵,制定更精準的個人化診斷與治療策略,進而降低不必要的藥物使用與治療成本,並減少高劑量、長期或持續性用藥所可能帶來的副作用。

特發性肺纖維化(IPF)臨床上具有高致死率且目前無法完全治癒。雖然泛組蛋白去乙醯化酶抑制劑(Pan HDAC Inhibitor)顯示出延緩病程的潛力,但其因缺乏選擇性而常伴隨嚴重的肝臟與皮膚毒性。為解決此一臨床瓶頸,本研究與本校藥學系余兆武教授團隊合作,成功設計並合成了多種具選擇性的新穎 HDAC4、HDAC5、HDAC6、HDAC7、HDAC8 或雙重 HDAC6/8 抑制劑,旨在透過精準的表觀遺傳調控,降低脫靶效應並提升治療安全性。

為了從中高效篩選出具備臨床潛力的藥物,本團隊建立了一個創新的三階段藥物篩選平台,系統性地加速藥物開發並降低成本。第一階段利用小鼠胚胎纖維母細胞(NIH-3T3)進行 TGF-1 誘導纖維化的初步篩檢,成功從多個小分子中篩選出命中化合物(Hit Compounds);第二階段則進一步使用人類肺纖維母細胞(HPF)進行療效驗證,並同步導入 Caco-2 細胞通透性與肝微粒體穩定性等早期藥代動力學(DMPK)評估,從中精準收斂出最後2個具高穩定性與穿透力的領導化合物(Lead Compounds);第三階段則透過molecular docking(Bleomycin)誘導的肺纖維化小鼠模型進行體內(in vivo)確效。

研究結果證實,我們團隊有找到關鍵的領導化合物能顯著抑制 TGF-1 誘導的肺纖維細胞增殖,並有效下調纖維化相關蛋白的表現,成功減緩小鼠體內的肺纖維化病程。此一結合「化學合成、體外多階療效與藥動學篩選、體內疾病模型確效」的完整藥物開發管線,不僅為選擇性 HDAC 抑制劑在抗纖維化藥物的應用上奠定了關鍵基礎,未來更有望開發出高效且無毒副作用的創新標靶藥物,切實改善特發性肺纖維化患者的生活品質。
