游舒涵副教授实验室(The S.H.Y. Lab) 主要研究主轴包括肿瘤免疫微环境(tumor immune microenvironment, TIME)与个人化医疗(personalized medicine)。本实验室运用高通量空间转录体技术,结合 AI 辅助多重免疫组织化学分析(AI-guided multiplex immunohistochemistry),解析肿瘤微环境中的空间组成与细胞互动特征。透过此平台,可重建肿瘤微环境中具功能意义的免疫微区(immuno-clustering),并进一步分析这些区域内不同细胞族群之间的空间邻近性与交互作用,进行高分辨率的空间生物学研究。

肿瘤免疫微环境由肿瘤细胞、肿瘤浸润性免疫细胞、癌症相关纤维母细胞、肿瘤血管系统及细胞外基质等多种成分共同构成。这些细胞与非细胞成分可协同调控肿瘤免疫逃脱、促进肿瘤生长与转移,并影响病人对治疗的反应。因此,针对病人肿瘤组织及其周边组织中的免疫细胞组成与空间分布进行系统性分析,将有助于建立标准化的肿瘤免疫微环境分析平台,并进一步应用于精准医疗与个人化治疗策略的发展。

游博士实验室所建立的肿瘤微环境研究平台,可应用于犬猫肿瘤及相关疾病治疗机转之探讨,也可整合于实验室长期关注的口腔鳞状细胞癌(oral squamous cell carcinoma, OSCC)研究。借由分析 OSCC 病人肿瘤微环境的空间组成与免疫细胞互动模式,本实验室致力于评估其作为免疫治疗疗效预测指标的潜力。相关研究成果将有助于临床医师依据不同病人的肿瘤免疫微环境特征,制定更精准的个人化诊断与治疗策略,进而降低不必要的药物使用与治疗成本,并减少高剂量、长期或持续性用药所可能带来的副作用。

特发性肺纤维化(IPF)临床上具有高致死率且目前无法完全治癒。虽然泛组蛋白去乙醯化酶抑制剂(Pan HDAC Inhibitor)显示出延缓病程的潜力,但其因缺乏选择性而常伴随严重的肝脏与皮肤毒性。为解决此一临床瓶颈,本研究与本校药学系馀兆武教授团队合作,成功设计并合成了多种具选择性的新颖 HDAC4、HDAC5、HDAC6、HDAC7、HDAC8 或双重 HDAC6/8 抑制剂,旨在透过精准的表观遗传调控,降低脱靶效应并提升治疗安全性。

为了从中高效筛选出具备临床潜力的药物,本团队建立了一个创新的三阶段药物筛选平台,系统性地加速药物开发并降低成本。第一阶段利用小鼠胚胎纤维母细胞(NIH-3T3)进行 TGF-1 诱导纤维化的初步筛检,成功从多个小分子中筛选出命中化合物(Hit Compounds);第二阶段则进一步使用人类肺纤维母细胞(HPF)进行疗效验证,并同步导入 Caco-2 细胞通透性与肝微粒体稳定性等早期药代动力学(DMPK)评估,从中精准收敛出最后2个具高稳定性与穿透力的领导化合物(Lead Compounds);第三阶段则透过molecular docking(Bleomycin)诱导的肺纤维化小鼠模型进行体内(in vivo)确效。

研究结果证实,我们团队有找到关键的领导化合物能显着抑制 TGF-1 诱导的肺纤维细胞增殖,并有效下调纤维化相关蛋白的表现,成功减缓小鼠体内的肺纤维化病程。此一结合「化学合成、体外多阶疗效与药动学筛选、体内疾病模型确效」的完整药物开发管线,不仅为选择性 HDAC 抑制剂在抗纤维化药物的应用上奠定了关键基础,未来更有望开发出高效且无毒副作用的创新标靶药物,切实改善特发性肺纤维化患者的生活品质。
